NEUROLOGICAL & BALANCE REHABILITATION · STAFFORD, VA
The Gap Between The Hospital And The Clinic
Leaving inpatient rehabilitation is a milestone, but it doesn't mean recovery is complete. For some families, the next challenge is figuring out how to continue making progress when getting to an outpatient clinic is difficult, home health eligibility is uncertain, or everyday life at home looks very different from the rehabilitation setting.
Having worked in inpatient rehabilitation, Dr. Paige understands how much effort goes into regaining the ability to walk, transfer, navigate stairs, and perform daily activities. The goal is to help carry those gains into the home environment and identify the right support for what comes next.

THE FIRST QUESTION
What Happens After You Come Home From the Hospital or Inpatient Rehabilitation?

For many families, the transition home can create an unexpected gap in rehabilitation. Some patients return home directly after an acute hospital stay, while others complete additional inpatient rehabilitation before discharge. In either situation, they may still need physical therapy but face challenges getting to an outpatient clinic, arranging transportation, or qualifying for home health services.
Mobile outpatient physical therapy can help bridge that gap by bringing individualized rehabilitation into the home when it is clinically appropriate. At Divine Physical Therapy, care is centered in Stafford County, Virginia, with limited availability in surrounding areas.
The first weeks at home are an important part of recovery. The environment has changed, structured rehabilitation may have decreased or stopped, and everyday activities can present challenges that were not fully apparent in the hospital. Fatigue, reduced confidence, mobility limitations, and fall risks may make it difficult to maintain or build upon recent progress.
Continuity matters. Whether through home health, mobile outpatient physical therapy, or clinic-based rehabilitation, the goal is to establish an appropriate plan that helps patients continue practicing essential skills, rebuilding strength, and gaining confidence in their own environment.
EXPERIENCE THAT CONNECTS BOTH SIDES OF RECOVERY
Understanding the Transition From Hospital to Home
Dr. Paige continues to practice in inpatient rehabilitation while providing mobile physical therapy through Divine. Her experience in both settings offers firsthand insight into the progress patients make before discharge and the challenges they face at home.
This perspective shapes her approach to helping patients maintain functional gains, navigate everyday activities, and work toward greater independence. Getting home is a milestone. Continuing recovery is the next step.

THE STRUCTURAL GAP
What Is The Gap - And Why Does It Exist?
Discharge from a hospital or inpatient rehabilitation facility doesn't mean recovery is complete. Some patients qualify for home health services, while others are ready for outpatient rehabilitation but still face challenges traveling to a clinic.
Between medical eligibility, transportation barriers, and the realities of returning home, families may find themselves unsure how to continue therapy.

The gap often becomes clear once everyday life resumes. Walking through the house, navigating stairs, managing fatigue, and safely completing daily activities may be more challenging than anticipated. Without an appropriate plan for continued rehabilitation, hard-earned progress can become difficult to maintain.
“I've seen how much effort patients put into regaining their independence during rehabilitation. I've also seen how challenging the transition home can be. My goal is to help bridge that gap, carrying the progress made in rehabilitation into the activities that matter most at home.”

THREE CARE OPTIONS, DIFFERENT REQUIREMENTS
Home Health, Outpatient, or Mobile PT: Where Does Divine Fit?
After a hospital stay or inpatient rehabilitation, the right next step depends on medical needs, mobility, insurance eligibility, and access to care.
These services are not competing options, nor does everyone need them in the same order. Some patients benefit from home health first, while others are ready for outpatient rehabilitation but have difficulty traveling to a clinic.
Divine Physical Therapy helps bridge that transition by bringing individualized outpatient care into the home when clinically appropriate and when payment eligibility allows.
HOME HEALTH
Skilled Care for Eligible Patients at Home
Provided through a home health agency for patients who meet eligibility requirements. Medicare-covered home health generally requires homebound status, a qualifying skilled need, and other coverage criteria.
CLINIC-BASED OUTPATIENT
Rehabilitation at a Therapy Facility
Patients travel to a clinic for physical therapy, with access to specialized equipment and resources when needed.
MOBILE OUTPATIENT
Outpatient Physical Therapy That Comes to You
Skilled outpatient rehabilitation delivered in the patient's home without requiring homebound status. Care can focus on walking, balance, transfers, strength, and everyday mobility in the environment where those skills matter most.
WHERE DIVINE FITS
Personalized Care During the Transition Home
Divine offers 75-minute, one-on-one mobile outpatient physical therapy sessions for eligible patients who need continued rehabilitation but prefer or have difficulty accessing clinic-based care.
Important Medicare Note: Divine is a direct-pay practice and is not enrolled in Medicare. Medicare beneficiaries cannot privately pay Divine for Medicare-covered outpatient physical therapy services. Eligibility is reviewed before scheduling.
PROTECTING FUNCTIONAL PROGRESS
Why Does Continued Rehabilitation Matter After Discharge?
Regaining strength, balance, mobility, and independence takes time and consistent practice. Whether someone is returning home after an acute hospital stay or weeks of inpatient rehabilitation, the skills gained during recovery still need to be reinforced through everyday movement.
At home, structured therapy often decreases, fatigue can increase, and familiar activities may present new challenges. Without continued practice and appropriate support, patients may experience setbacks in strength, endurance, confidence, or functional mobility.
Recovery doesn't stop at discharge, and neither should the opportunity to build on progress. Walking through the house, navigating stairs, getting in and out of bed, and completing daily routines all provide opportunities to reinforce meaningful movement.
The goal is to turn progress made during rehabilitation into greater confidence, safety, and independence at home.
PERSONALIZED REHABILITATION AT HOME
What Does Mobile Rehabilitation Look Like in Your Own Home?
Recovery begins with a comprehensive assessment of how you move, feel, and function in your own environment. Dr. Paige considers everything from strength, balance, mobility, and movement quality to the everyday challenges of navigating stairs, transferring from furniture, and moving safely throughout your home.
Your treatment is never limited to functional exercises alone. Each 75-minute, one-on-one session is tailored to your needs and may incorporate skilled manual therapy, targeted soft tissue treatment, joint mobility techniques, neuromuscular re-education, progressive strengthening, and balance or gait training when clinically appropriate.
Family and caregiver education is thoughtfully integrated into care, including safe assistance techniques, strategies to encourage independence, and ways to support continued progress between visits.
The difference is in the details: unhurried attention, hands-on treatment when beneficial, and a rehabilitation plan built around your body, your goals, and the environment where you live.

PAYMENT & ELIGIBILITY
A Word About Eligibility And Money
Divine Physical Therapy is a direct-pay practice and is not enrolled in Medicare. We are unable to provide Medicare-covered physical therapy services to Medicare beneficiaries on a private-pay basis.
If you or your family member has Medicare, please discuss continued rehabilitation options with your discharge team or a Medicare-enrolled physical therapy provider.
For individuals with other coverage or self-pay arrangements, a complimentary Discovery Consultation can help determine whether Divine's mobile services may be an appropriate fit.
FOR FAMILIES PLANNING A DISCHARGE
Four Steps Toward a Smoother Transition Home
01. Plan Before Discharge
Start the conversation early. Review your loved one's mobility needs, recommended rehabilitation, equipment, and available care options before returning home. If considering Divine, a complimentary Discovery Consultation can help clarify whether mobile outpatient PT may be an appropriate fit.
02. Prepare the Home
Consider entry steps, walking paths, bathroom accessibility, furniture heights, and necessary equipment. A personalized home assessment can identify challenges that may not be obvious until everyday routines resume.
03. Keep Recovery Moving
Follow the discharge team's recommendations for continued therapy and safe activity. Whether through home health, mobile outpatient care, or a clinic, consistent and appropriate rehabilitation helps patients build progress and work toward greater independence.
04. Progress With Purpose
As strength, mobility, and confidence improve, rehabilitation should evolve. Dr. Paige continually reassesses goals, adjusts treatment, and recommends a transition to another setting or level of care when appropriate.
