Neck & Shoulder Pain: One Of Dr. Paige's Favorite Problems To Solve
Neck and shoulder problems rarely stay inside one structure. The neck, shoulder blade, ribcage, rotator cuff and upper back all influence how the arm moves, and sometimes age, hormones, work demands, sleep or the rest of the trunk matter too. This is a region where Dr. Paige especially enjoys combining hands-on treatment with movement analysis and progressive strength.

THE RIGHT QUESTION FIRST
Is My Shoulder Pain Coming From My Neck, Or Is My Neck Reacting To My Shoulder?
Either can happen. The nerves that supply the shoulder and arm begin in the neck, while shoulder motion depends on the shoulder blade, ribcage and upper back. A good examination does not assume the painful spot is the only problem.
That is exactly why Dr. Paige enjoys this region: manual therapy can help clarify what changes symptoms, and the response can then be carried directly into shoulder, scapular, thoracic and whole-body strengthening during the same 75-minute visit.

DIFFERENTIAL EXAMINATION
How Do You Tell A Neck Problem From A Shoulder Problem?
By behavior, not by where it hurts. A true shoulder problem is usually provoked by shoulder movement and load - reaching, lifting, lying on it. Neck-referred pain tends to change with head and neck position, may travel below the elbow, and often brings tingling or altered sensation along for the ride. The examination reproduces your symptom deliberately, from each candidate source, until the driver confesses.

"Neck and shoulder care is where the Divine Restore & Reset Movement Method™ shows its full range: differential examination, myofascial and joint-based manual therapy, dry needling when appropriate, scapular and rotator-cuff loading, thoracic mobility and functional strength. The shoulder that is actually a neck usually gives itself away in the first ten minutes - the “shoulder” pain that shifts when I move a client's head, arm symptoms no shoulder structure can anatomically explain, strength that returns the moment the neck is repositioned. I check every shoulder from the neck down and every neck to the fingertips, because the most expensive mistake in this region is six weeks of well-executed exercises aimed at the wrong joint.

BIOMECHANICS & LOAD TRANSFER
Rotator Cuff Pain: Why The Shoulder Blade Runs The Show
The rotator cuff is four small muscles that keep the ball centered on a socket that itself sits on the shoulder blade - so the cuff can only work as well as the platform beneath it moves. Restricted thoracic mobility and altered scapular mechanics are frequent upstream drivers of cuff irritation, which is why effective rehab loads the mid-back and shoulder blade, not just the sore tendon.
Every degree of overhead reach the stiff mid-back refuses to contribute has to be borrowed from somewhere, and the somewhere is usually the front of the shoulder - which is why a cuff tendon can stay irritated indefinitely while it is dutifully strengthened, until the thoracic spine and shoulder blade start paying their share of the movement again.

First, partial tears show up on imaging in plenty of people with no symptoms at all, so a tear on a report is a finding, not a verdict - most respond well to rehabilitation, with surgery reserved for large traumatic tears or genuine failure of conservative care. Second, progress here is tracked with the DASH questionnaire and strength retests rather than impressions, so you will know whether the plan is working, not just hope so. For shoulders recovering from an actual repair, the post-operative pathway lives on the post-surgical rehabilitation page.

CLINICAL INSIGHT & BIOLOGICAL CONTEXT
Frozen Shoulder: Why Midlife Women Deserve A Closer Look
Frozen shoulder, or adhesive capsulitis, causes progressive pain and loss of both active and passive shoulder motion. It most often appears in midlife, and women are disproportionately represented. A major review reported that approximately 70% of patients with adhesive capsulitis are women, with most cases occurring between ages 40 and 59. Le et al., 2017.
That age pattern has raised an important women's-health question: could the hormonal changes of perimenopause and menopause influence the connective-tissue biology of frozen shoulder? Estrogen has anti-inflammatory and antifibrotic effects in several tissues, so declining estrogen is biologically plausible as one contributor - but the clinical evidence is still emerging, not settled.
A 2026 pilot study of 1,952 menopausal women found adhesive capsulitis in 7.65% of women not using hormone therapy versus 3.95% of women using hormone therapy. The estimated odds were nearly two times higher without hormone therapy, but the result was not statistically significant (OR 1.99; 95% CI 0.86-4.58; p=0.10). The researchers concluded that larger prospective studies are needed before hormone therapy can be considered protective. Reinke et al., 2026.
Divine does not diagnose an estrogen deficiency from shoulder pain or suggest hormone therapy as a shoulder treatment. The useful takeaway is broader: a woman in her 40s, 50s or 60s with a progressively stiff shoulder deserves an assessment that recognizes age, diabetes, thyroid disease and potentially menopausal context as part of the history rather than treating the shoulder as if it exists in isolation.
Rehabilitation still focuses on the shoulder in front of us: managing irritability, maintaining and gradually restoring motion, preserving strength, and progressing load according to symptoms and stage. The hormonal discussion adds context; it does not replace orthopedic examination or appropriate medical care.
EPIDEMIOLOGY
Who Is Neck Pain Affecting Most?
Neck pain is not a niche problem. The Global Burden of Disease 2021 analysis estimated that about 203 million people worldwide were living with neck pain in 2020.
~203 million
People worldwide living with neck pain in 2020.
2,890 vs 2,000
Age-standardized prevalence per 100,000: women versus men.
Ages 45 to 74
Where prevalence peaked, in both sexes.
That does not make neck pain a "women's condition," and population statistics do not explain one individual's pain. They do show that midlife and older women carry a disproportionate burden, worth knowing walking into an evaluation, not a diagnosis in itself. GBD 2021 Neck Pain Collaborators, 2024
ERGONOMICS & DAILY LOAD
What About Desk Posture And Sleep?
Posture contributes to neck pain, but it is rarely the sole cause - sustained positions of any kind become uncomfortable, which is why the most reliable intervention is changing position often rather than holding a perfect one. Strength, sleep quality, stress and total daily load shape neck symptoms at least as much as the angle of your monitor does.
Night pain deserves its own sentence: side-lying compresses an irritated cuff or bursa, which is why shoulders so often hurt most at 2 a.m. Pillow strategy helps quickly; persistent severe night pain gets screened rather than assumed. And where trigger points in the upper trapezius and neck are feeding pain or tension-type headaches, dry needling is folded into the session - consented, included, and always in service of the loading work that follows. When the neck pain is really a whole-spine story, it is treated as one - the same driver-first logic as on the back pain page.
THE SHAPE OF A BACK PAIN PLAN
Four Phases, Finished Properly.
The Desk Decade
Neck tightness, creeping headaches, a shoulder that aches by Thursday. Load management plus strength, not posture-shaming.
The Overhead Athlete
Swimmers, throwers, lifters: cuff irritation with a mid-back and shoulder blade that stopped contributing to smooth, efficient movement.
The Waking Shoulder
Fine all day, on fire at night. Usually cuff or bursa compression, and very treatable once the driver is confirmed.
The Referred Neck
"Shoulder" pain with tingling past the elbow that no shoulder test reproduces. The neck gets examined first here, not last.
