MANUAL THERAPY & DRY NEEDLING · STAFFORD, VA
Hands-On Treatment, And When Needling Belongs In It
Skilled hands change what a body will allow, for a while. What you do inside that while is what your body keeps. This area of care holds every hands-on tool Dr. Paige uses, including nearly a decade of certified dry needling, and the philosophy that decides when each one earns a place in your session.

THE BASICS
What Is Manual Therapy?
Manual therapy is skilled, hands-on treatment (joint mobilization, myofascial release, instrument-assisted soft tissue work, cupping and dry needling) used to reduce pain and restore motion so that exercise can make the change permanent. At Divine Physical Therapy, hands-on care is incorporated when clinically appropriate within one-on-one mobile sessions, primarily throughout Stafford County, Virginia, with select nearby visits considered by arrangement.

Nothing here is billed as an add-on. Every technique, needling included, is part of the session when your examination says it should be, a pricing choice that keeps the clinical decision clean. HSA and FSA cards may be used when your plan permits.

Hands-on treatment attracts more mythology than any other part of physical therapy. Here is what each tool actually does, and the rule that decides when it's used. Reviewed by Dr. Thea Paige, PT, DPT, CMTPT, PCES (about Dr. Paige).

THE HONEST LIMIT
Is Hands-On Treatment Enough On Its Own?
No, and a practice that says otherwise is selling repeat visits. Manual therapy reliably reduces pain and improves mobility in the short term, and those effects fade unless the new range is loaded and used. That is not a weakness of the technique; it is the reason every hands-on minute here is paired with movement in the same session.
Hands-on treatment works mostly by changing what the nervous system will permit: pressure, glide and needling reduce a region's protective guarding, briefly widening the range and load it tolerates. That widened window is the treatment's product, and loading inside the window is what converts it into something your body keeps.
How Is This Different From Massage?
Massage therapists are skilled clinicians of soft tissue, and many of our clients see one regularly; the two are complementary, not competing. The difference is the framework: manual therapy here follows a diagnostic examination, targets the specific joints and tissues that examination implicates, and is always coupled to loading and retraining. You are not booking hands; you are booking a clinical decision about where hands will change something.

INCLUDED, NOT UPSELL
Is Dry Needling Included In A Session?
Yes, when it is clinically appropriate and you have given written consent, dry needling happens inside the standard 75-minute session at no additional charge. It is never sold as a separate service, an upgrade, or a package. And if your first question is how it differs from the other needle discipline, dry needling vs. acupuncture answers it in full, respectfully, in both directions.
WOULD DRY NEEDLING BE APPROPRIATE FOR ME?
A Quick Pre-Screen, Not A Clearance.
Dry needling is optional. This quick screen can help you identify information Dr. Paige should know before a needle is considered. It does not clear you for treatment and it does not replace the clinical screening and written informed consent required before dry needling.
Choose the answer that best describes you today. A “review first” result does not automatically mean you cannot receive dry needling.
01. Do you currently have an active skin infection, open wound, unexplained redness/drainage, or significant skin irritation in the area that might be treated?
02. Are you currently feverish, acutely ill, or being treated for a systemic infection?
03. Do you take a blood thinner, have a bleeding/clotting disorder, bruise very easily, or have been told you have low platelets?
04. Are you pregnant, could you be pregnant, or actively trying to conceive?
05. Have you had lymph nodes removed, been diagnosed with lymphedema, or had significant swelling problems in the area?
06. Is there a recent surgical site, joint replacement, other implanted medical device, cosmetic implant, filler, or other implanted material near the area you want treated?
07. Do you have diabetes with poor wound healing, significant immune suppression, or another condition that affects healing or infection risk?
08. Have you ever fainted, become very lightheaded, or had a strong vasovagal reaction with needles, injections, blood draws, or medical procedures?
09. How do you feel about needles?
10. If electrical stimulation might be used with needles, do you have a pacemaker, implanted electrical device, or another condition your clinician has told you may be affected by electrical stimulation?
This is not medical clearance. Dry needling remains optional. Dr. Paige screens for precautions and contraindications, discusses risks, benefits and alternatives, and obtains written informed consent before treatment. A “review first” answer may simply mean the technique, body region, depth, positioning, or timing needs to be modified — or that another treatment is a better choice that day.
Virginia classifies dry needling as an advanced physical therapy procedure requiring additional postgraduate training. The physical therapist must practice within the level of that training and obtain informed consent that includes the risks and benefits before performing dry needling.
“I include needling at no extra charge on purpose. The moment a needle carries its own price tag, someone in the room has an incentive to use it, and that someone is never the client. Folding it into the session means the only reason a needle ever comes out is that your examination earned it. Clients feel the difference in that, even before they feel the needle.”
SCREENING, ALWAYS
Who Should Not Receive Manual Therapy Or Needling?
Hands-on techniques are modified or set aside where tissue cannot safely take them: over active infection, unhealed wounds or recent fractures, with certain bone and circulatory conditions, and wherever your history or preference says no. Needling carries its own specific list (bleeding disorders, lymphedema, pregnancy considerations and more) which is covered fully, and first, on the dry needling page. Screening happens at evaluation and again on the day; declining any technique never limits the rest of your care.
Manual therapy is one part of the full range of physical therapy services at Divine Physical Therapy, and because the same clinician provides all of them, the hands-on work is never separated from the plan it serves.

The freed range is immediately trained: strength and control work while the nervous system is willing.
Send It Home
A short home program asks the tissue to keep what it gained, because the session ends, and the week doesn't.
